If a baby has persistent vomiting after most feeds, along with irritability, crying during or after feeds, poor weight gain, disturbed sleep, back arching, forceful/projectile vomiting, green or yellow vomitus, or recurrent cough and cold associated with feeds, we consider the possibility of gastroesophageal reflux disease (GERD) and the baby should be evaluated by a paediatrician.
However, many babies are “happy spitters.” They may bring out small amounts of curdled milk after feeds but are otherwise feeding well, comfortable, active, and gaining weight normally. In such cases, it is usually physiological reflux, which is very common in young infants and tends to improve as the baby grows.
You can try the following measures:
* Burp the baby well after every feed
* Keep the baby upright for 20–30 minutes after feeding
* Give smaller, more frequent feeds
* Avoid overfeeding
* Ensure the baby is latched properly during breastfeeding
In many infants, simple reflux improves gradually as the digestive system matures. Therefore, if the baby is otherwise well, you may observe for improvement before starting medications. If your doctor has already prescribed treatment because significant symptoms are present, continue the medicine as advised and use the liquid formulation prescribed for infants rather than tablets.
