You do need help. The answer to your last question that wether you "need to seek some help" is a loud and bold YES.
Symptoms of panic attacks, now leading into significant depression, anxiety, impulsively, feelings of loneliness and being deserted, and ideas of self harm are all significant indicators of you needing psychiatric and mental health care.
It is very nice that you have chosen this platform as a first step towards seeking help, and I would strongly recommend and urge you to seek a consultation with a psychiatrist near you at the earliest. Keeping your condition untreated is dangerous and can lead to complications.
In the meantime I will suggest that you please stay around people who care for you like family and friends, talk about your problems with these persons, keep in mind the factors which stop you from taking this step, take up se light physical exercise like walking or jogging, engage in some pursuit of your choice, etc.
However do not delay seeking help..
