Hello. Your report is of 2013. Now in 2017 you need to take a fresh x-ray and Mri of your spine and consult an orthopaedic.
Also I recommend you to get few appropriate exercises from a mckenzie practicing physiotherapist at the earliest so that you are mobile and can prevent any further issue.
You should also do a lot of anom velom and deep breathing exercises to help you step up your health faster.
