Foot/ankle pain

2 foot pain attacks before. I went to the ER second attack, dismissed it has a sprain when no injury. They did check for a break or blood clot found none. This is third attack, the foot pain swelling about the same as before, the last 24hrs I now have an intense throbbing warm pain in my ankle/heel. It is on the outer part of my foot. I have tried elevating it only worsens. I now have pain and throbbing in my calf moving to the knee. Barely can walk. I do have CHF, on bp and diuretic meds.
