A small dot-like opening in the wound area after kshar sutra and fissure surgery can be a healing scar dimple, a residual sinus track, or the external opening of a recurrent fistula. Since you have no bleeding or discharge and only mild pain, it may well be scar tissue rather than an active fistula, but that can only be settled by an examination and imaging, not by looking at it from a distance.

Please get reviewed by a colorectal or general surgeon with an MRI fistulogram of the perianal region; this will clearly show whether any track remains and would avoid unnecessary repeat surgery. Until then, keep stools soft with a high fibre diet, plenty of water and a stool softener such as isabgol, take warm sitz baths twice daily, avoid straining, and keep the area clean and dry. Also get haemoglobin, blood
sugar and a basic nutritional check done, as weight loss and weakness after repeated surgeries need correction.
