Is filling possible in this...I don't want to go with rct first... Symptoms are...pain while tight chewing and cold sensitivity..that is very brief

Deep carious lesion, and it looks relatively close to the pulp Filling may be possible and you do not need to start with Rct. Pain while tight chewing and cold sensitivity occur due to deep cavity and periodontal ligament inflammation.
Next Steps
If cold pain lingers for 30+ seconds, occurs spontaneously/night time,or is getting worse,Rct becomes much more likely

Hlo sir….as i can see the cavity is very close to pulp and that’s why u r experiencing pain while chewing and sensitivity while having cold food…. I will suggest u should go for rct as filling will not give you permanent solution,it will fail and u will be needing rct anytime soon.So better to go with rct only. As the cavity is very close to pulp and it might be possible if u will go for filling the pulp will get exposed while excavating the cavity and u will be needing rct only.

We can try for filling with pulp capping after cleaning the cavities, and temporary filling. If there is no pain in three days after filling we will do permanent filling but if there is pain we will go for RCT.

If you are having pain on chewing and having sensitivity to cold stimulus, chances are your cavity has already reached pulp and causing this reaction. Moreover the tooth apex also needs to be examined, and it is not clearly visible in the xray. Even if the tooth gets restored, it might trigger an inflammatory response due to pulpal irritation and you might have to go for root canal.
Next Steps
Best option would be to go for the root canal instead of spending on restoration first and then eventually going for root canal treatment. If you still persist on going for restoration, a gic restoration can be done temporarily and can be monitored for a while, then go for rct if pain continues.

be careful while chewing. do not eat hard foods from that side. rinse your mouth properly and do not let food lodge in that area.
