My baby is 2 years old. He has had a fever for the last 3 days, when we give paracetamol plus ibuprofen the fever gets normal and it again comes back. His nose is blocked. He is not eating anything only demanding water. His stomach is gassy and sometimes looks like he is about to vomit. We are giving him antibiotics, anticold syrup, antacid antivomit, .what to do in this case.

Recurring fever with blocked nose, poor appetite, and gas in a 2-year-old usually indicates a viral URTI. The fever returning after paracetamol/ibuprofen is common in viral illness. Continue alternating both for fever control and ensure good hydration (ORS/water). Don't stop antibiotics mid-course if prescribed. Watch for danger signs: persistent high fever beyond 5 days, rash, labored breathing, or extreme lethargy.
Next Steps
Get the child physically examined by a pediatrician if no improvement in 24–48 hours or if any danger signs appear. Consult me on Practo for further guidance.

Fever more than 3 days will require physical consultation. Mostly viral fever,  Plenty of water to drink.
