Toe nail has thickened and growing upwards, white crumbly, and I'm not sure if it's thickened, skin or nail within so I m careful not to cut directly, microscopy of nail clippings 1year ago showed fungus with hyphae. I have used fungal meds and  continue lacquer 2 plus years, but it has gotten worse, is there any treatment for nail other that topical or pills Please see pic , the inside part is softer, is this nail or skin, is this common and will it ever be back to normal Thanks for your time!
