Your story fits gastritis with functional dyspepsia and reflux - alcohol, especially beer, is the single biggest trigger here, and the fact that you are fine on home-cooked food and flare only with beer says it clearly. Stop beer completely for 8 weeks; that alone will change how you feel. Add pantoprazole 40 mg before breakfast with a prokinetic like domperidone or itopride before meals for 4-6 weeks, small frequent meals, no dinner within 3 hours of bed. For the chronic constipation, Zandu Nityam daily long term is not ideal - shift to isabgol or lactulose with 3 litres water and more fibre. Also get H. pylori tested (stool antigen or breath test, both avoid the endoscopy your gag reflex cannot tolerate), plus CBC, LFT, thyroid, and confirm Sartel H is not adding to the acidity.
Next Steps
Cut out beer, start the acid and motility medicines, get H. pylori and basic bloods done, and consult me on Practo or at Synergy Multispeciality Hospital, Miraj with the reports - if there is vomiting blood, black stools or weight loss we will need the endoscopy under sedation.
