An untreated Pilonidal cyst can form a sinus tract, which is a channel that opens from the cyst to the surface of the skin. There can be development of multiple sinus tracts. As you mention you are overweight, this can increase the depth of the buttock cleft and lead to more sweating and friction, making you a good candidate for this pilonidal cyst to worsen. So for precaution, opt for loose fitting underwear and pants to allow the screen to breath and prolonged sitting. Daily washing with mild soap is crucial and then makes the area dry.
Next Steps
It's better not to delay treatment. You may need I&D of the cyst and can use homeopathy to stop recurrence.

With an active pilonidal cyst, especially if painful or discharging, it’s best to avoid strenuous exercise that causes friction or sweating in the lower back/buttock area. Sweating and friction can worsen irritation or infection. For now, focus on gentle, low-impact activities like walking, stretching, or upper body workouts that don’t stress the tailbone area. Maintain hygiene, wear loose, breathable clothing, and consider warm compresses. Once the inflammation subsides, you can gradually increase physical activity. Weight management is still possible through a healthy diet and modified activity during recovery.

Homeopathic treatment can help address the root cause of your pilonidal cyst naturally, reduce inflammation, prevent recurrence, and support healing without surgery. Regarding exercise: While heavy workouts that cause excessive sweating or friction in the lower back area should be avoided during an active flare-up, you can still engage in gentle, low impact activities like yoga , brisk walks etc. Once the cyst is under control, you can slowly increase physical activity. Sweating itself isn’t the enemy friction, bacterial overgrowth, and lack of hygiene are bigger triggers. Managing those alongside your treatment is key.
