It is very unfortunate that you are facing this problem. It is not possible to suggest you which one to undergo (normal or surgical) without clinically seeing the teeth and X-rays. As to your confusion, I would say as there is delay in getting the treatment done we loose some tooth structure over time which makes it difficult to remove the tooth. Sometimes due to insufficient accessibility the tooth has to be removed surgically which is not a wrong method. The whole purpose is to remove the affected tooth and save your bone as much as possible. I hope this will solve your confusion and suggest you to take an option of an endodontist and oral surgeon.
