For a 4-month-old exclusively breastfed infant, passing stool only once every several days can be normal. Breastfed babies may sometimes go 5–7 days without stool if they are otherwise comfortable, feeding well, passing urine normally, and when stool comes it is soft.
Since this baby is feeding well with good urine output and no other concerning symptoms, constipation is not necessarily present.
If the baby remains comfortable and continues feeding and urinating normally, watchful waiting is reasonable.

Advice to the parents:
* Continue exclusive breastfeeding; do not routinely give water, honey, gripe water, or laxatives.
* Gentle tummy massage and bicycling of the legs can be tried.
* Observe for passage of stool; the consistency is more important than the number of days.
* Vaccination can occasionally temporarily alter bowel habits, but 5 days without stool is not by itself an indication of a vaccine complication.
Health Tips
Seek pediatric assessment urgently if there is persistent vomiting (especially green/bilious), abdominal distension, fever, poor feeding, lethargy, blood in stool, reduced urine, significant pain/irritability, or if stools are hard/pellet-like.
