If the illness has started only before 4 years ,the possibility of dementia should be considered.Dementia ,is a neuropsychological illness ,where the patient gradually lose their executive functions of attention ,concentration ,memory ,judgment ,mostly associated with behavioural disturbances.If the illness has started in her late twenties or early thirties ,diagnosis of mood disorder to b considered .For both the cases ,she needs to be on antipsychotic s for which quetiapine is a good choice.In both the cases ,patient may not understand their illness ,and may not b willing for treatment .Othet factors like blood
sugar ,bp etc has to b under good control to prevent relapses Medications must be continued ,unless otherwise asked to stop because of adverse effects.
