As per my opinion, it might be different for conditions like orthopedic, neurological or sports related injuries. Recovery phase varies in these conditions.

I would suggest an off day to rest and let the body recover, heal from the exercises. Even according to the guidelines of ACSM, there is a day or 2 of rest day in the exercise regime, be it cardio, strength training or weight loss.

During the gap period, eat good, nutritious food, get adequate sleep and do some stretching as a relaxation technique.

Doing daily physiotherapy not cause any harm specially if you are doing for fitness purpose. But if have any kind of pain which might be increased due to any kind of exercise you have to take rest of consult with therapist. Otherwise no need to take break once a week it's actually depends on your health condition

It depends!!! Acute stage 3 sessions during the 1st week, 2 or 3 sessions week after However here sometimes people rush through 7 sessions in 7 days. There is very limited evidence to that. Actually as the stages of healing has only crossed stage 1 in a week. So, as you see, it depends on the condition and many other factors.
