Cloudy and sort of oily urine ankle swel

Dear respected doctor, I m facing health issues since 8 months, my UTI coming back again and again , there is mild ankle swelling since 7 months I did all tests usg kub CT scan endoscopy colonoscopy and found little abnormalities like mild liver hypoattnuetion spleen enlarged(15cm) simple cortical cyst in left kidney and obe in right ovary 24 hour protein was 274 5 months back ..again I did urine culture and urinalysis and it was klesbiella pneumonia positive then I took some antibiotics for a week ,repeatedly both tests urine culture came negative but urinalysis shows pus cells 20 25 and some bacteria, I went to nephrologist for all of this, and he is saying there is nothing serious, as I m having very concentrated foamy urine, is he saying correct or do I need to do some test to diagnose the issue, any kind of help would appreciated...current hba1c is 5.7
