Chronic kidney disease with eGFR 19

My father is a diabetic with CKD. His eGFR is 19 and creatinine is 3 to 3.2... Is this indicate kidney failure? If not can eGFR can be increased?

If he his creatinine has been elevated for a long time then it is CKD. We need to do Urine routine UPCR Ultrasound KUB as well as blood tests like urea electrolytescalcium phosphorus. He needs a complete review with a Nephrologist to determine further treatment plan
