Child's got undescended testis

My brother's testis were normal at birth but both went up when he was 2 years old because of using diapers frequently. We took him to doctors and they prescribed medications to bring them back. Now he's 7 and the testis are still undescended. Can they be back by hormonal treatment? or is a surgery required??
