This is a very common problem. Don't worry. You have to undergo RCT in the affected tooth. Of course, first we have to see whether RCT is possible or not in that tooth. If it is not possible, then you have to get it extracted.

there could be 2 reasons. 1) the cavity could be deep, which requires filling/restoration
2) as you mentioned its bottom last tooth,so it could be gum infection if not pain from the cavity.
