Bowel pains and bleeding

I've been passing blood in my stool for over a year with bowel pains. My stool is constantly changing textures and every time I go it hurts my stomachs. It makes me feel sick and weak and it feels exhausting every time I go. The pain comes and goes but it has been like this for over a year. The amount of blood is getting more.
