Black patches on skin (hand & forehead)

I have black spot/patches on my hand, nose and forehead Seems like dead skin But the spots/patches are increasing daily My B12 and Vitamin D are on the lower side in a 4 months old blood report. Iam a non vegeterian & I drink and smoke daily. Please refer the attached pics.
