Black patch on left cheek

Hi dr I have developed black on ny left cheek. It is there from last few years. It pains when I move trimmer over that area. I am 99kg Male age 35 with h1bc1 = 7 i have started gym from last month and handling food to reduce sugar. But this patch is looking very odd now. Fist of all what is this patch or Dieases?? Should i try some medicine or go fo for laser.. Sharing best picture of that.. Please suggest...
