Occasional upward eye rolling in infants can be part of normal neurological development
Seen especially during sleep onset, feeding, or drowsiness
May appear more noticeable when baby is tired or overstimulated
If baby is otherwise active, feeding well, gaining weight, and meeting milestones, chances are high it’s benign
But still worth evaluating once to rule out neurological causes

Observe and record a video of the episodes if possible
Schedule a neurological evaluation to rule out subtle seizure activity (like infantile spasms or focal seizures)
Pediatric EEG or neuro consult may be needed if clinically indicated

Red flags:
Eye rolling is frequent, sudden, or rhythmic
Associated with stiffening, jerky movements, or loss of consciousness
Episodes last more than a few seconds or occur multiple times a day
Baby shows poor eye contact, developmental delay, or regression of milestones
Any family history of seizures
