An MRI fistulogram does not require an active abscess to show a fistula tract.
A good-quality pelvic MRI will normally pick up even a dormant, non-infected fistula tract because the tract is made of fibrous tissue it doesnât disappear simply because the abscess is inactive.
So if your repeated MRIs over the last 6 months are showing no fistula tract, this is actually a very reassuring sign.
However, symptoms like pain, discharge, swelling, itching, or repeated infections can sometimes happen even when the main tract has healed. These may be due to:
A previously healed fistula site that is still sensitive
Residual inflammation
Anal fissure
Skin infection
Recurrent constipation or IBS
Pelvic floor muscle spasm
Post-surgical scar tissue
These conditions can mimic fistula pain, so itâs important to evaluate them properly.
