Abdomen throat and weight problem

Sir/madam, I am feeling goiter under my throat, while walking small distance my heart(lungs) is getting tired.iam gaining weight, even iam eating less quantity of food.i feel my body is stretching some times and some times normally.frequently iam feeling that somebody is handling my neck hardly.my foot is swelling when I am trying to walk for 20-30 minutes.pls tell what is my problem and how can I get back myself healthier lifestyle.i want to be healthy because I have two small kids .my husband is died due to cancer in last February.
