Go for rabies vaccine as the nail scratch  is poisonous.  I learn it during  my training period in PREVENTIVE N SOCIAL MEDICINE. 

No risk ofherpes, T.b., hiv or any such major infection. Clean the wound very well  with antiseptic solution and apply povidone ointment over it.

Clinically, the risk of transmitting serious blood-borne infections like HIV or STDs through a simple fingernail scratch is virtually zero. For such infections to spread, there must be a direct exchange of infected blood or body fluids into an open wound or mucous membrane. Fingernails do not 'carry' or 'store' these viruses in a way that can cause infection through a superficial scratch. While you mentioned being in 'high-risk environments,' unless your nail was actively soaked in infected blood just seconds before the scratch, there is no medical basis for worry regarding HIV, Herpes, or TB in this scenario."
