Why is iui failing?

Hello, I've patent tubes (hsg done), i am tracking ovulation through follicular Study and ovulating on time, endometrial is always 9-9.5mm on the day of ovulation . still 2 iui failed for me. What could be the reason?

You need  to  do few more cycles of IUI,to say it's failing
