The problem is in the tooth so no need to visit an ENT specialist. According to your history and x rays i can see periapical infection in your tooth. So you have to go for a root canal treatment or extraction. Crack cannot be visible in a x ray most of the times and so cannot rely on x ray. Physical tests are more appropriate in diagnosing cracks. If you want to save your teeth first undergo a root canal treatment and wait for ten days if pain completely subsides then go for a cap. If still pain persists you have to go for extraction pr else you can directly go for an extraction and replacement of teeth. Choice is yours.
