Most of the unilateral vocal cord paralysis are idiopathic if there is no history of and neck surgery. Other causes include any lesions related to lungs, mediastinum, and some lesions of heart such as mitral stenosis. CT SCAN required to rule out such causes. Idiopathic palsy is usually diagnosis by excluding all other causes. Usually its harmless and slowly other vocal cord will compensate and voice will become almost normal
