Urticaria n swelling

Suffering from urticaria with swelling beneath it, on the right arm. It started a week ago. Urticaria appears one after the other. It itches violently. And disappears in a day or 2 but a new one appears. It started near the elbow then the new ones appeared on a distal position. Now it is on the right ring finger plus previous one is near the wrist. Please suggest the treatment and also what's the diagnosis. Thank you.
