Urinary Tract infection

I had burning urine sensation from few days . I did my complete urine examination & urine culture sensitivity test. In CUE it showing 40 pus cells, 25 RBcs and protein + . In the culture sensitive test it's showing Enterococcus Fiecium 10000 CFU/ML.some medication also prescribed in the report I am attaching it. Please advice me which tablet I have to use and how many days I have to take it. Please advice sir. Thank you
