Understanding prescription for my Pregna

I am now 6 weeks pregnant. My doctor has shared a prescription for all the medicines/tests I need to do. I need to understand from the prescription whether I take the tetanus shot now and after 4 weeks or I take it after 12 weeks. Also the blood sugar and other tests need to be done after 12 weeks? Kindly help in understanding this.

First shot of tetanus now and second dose after 4 weeks Blood tests after 4 weeks .
