Tsh 12 but symptoms of hyperthyroidism

Hello I have been diagnosed and been treated for pcod since a year and since last few months, my periods are regular. this month, i missed my period by 2 weeks. from mya week before my regular period date, I had fever like state and weakness and just below my chin, there was swelling..like double chin (it has been swelling there from last 4 months). so i got thyroid test..where tsh was 12 but t3 t4 normal. After 1 week of missing period, i started having period like cramps, more recently, i feel very hot, having vertigo since 2 days, and woke in middle of night today and cant fall back to sleep. i did urine test of pregnancy but it was negative. what shall i do? what is wrong with me?
