Hypothyroidism
Myxedema = severe hypothyroidism.
SX â weakness/fatigue, feel cold, dec memory, constipation, weight gain, depression, bradycardia, dec DTRs.
PE â all (look for weight gain, dry skin, loss of hair, macroglossia, goiter, non-pitting edema (myxedema).
Next Steps
DX â TSH (increased), Free T4 (decreased). Antibodies against thyroid peroxidase (positive in Hashimotoâs Thyroiditis). ⢠CBC, CMP, fasting lipid panel.
Health Tips
TX â maintain a euthyroid state, w/ F/U Q6 wks. ⢠Levothyroxine, start 1.6 mcg/kg PO QD. Then adjust by 12.5-25 mcg/day q4-6wks. ⢠2/3 the dose in elderly.
