A TSH of 10.24 suggests under-replacement or reduced absorption of levothyroxine; the dose should not be changed blindly without FT4 and clinical review. Take it fasting with water 30–60 minutes before breakfast, keeping iron or calcium 4 hours apart. Please repeat TSH with FT4 and arrange a review for dose adjustment, especially if you are planning pregnancy. You can book a consultation with me on Practo for a complete review.
Next Steps
Continue the prescribed tablet as advised, repeat TSH and FT4 in 6–8 weeks, and review the reports before any dose change.
