First thing first.. get it examined by a surgeon and get relevant investigations done. Since the age is 54 years nothing can be ruled out with just this information. And any swelling in scrotum should be taken seriously and should be left as such for 5years. Get ultrasound scrotum as well as abdomen done. Blood investigations. You can consult me i am a general surgeon.
Next Steps
ultrasound scrotum and abdomen. and blood investigations
Health Tips
get it done the next morning itself

Cancer can't be present at the same stage for 5 years sir .. very very unlikely. Get a USG scrotum done. Why the confusion. Get it done and review with me if you have any doubts (I am a surgeon )
