Stressed, forgetful and tired

I am constantly forgetting things,and muddling information up especially for appointments and work. I am 19 and am very worried that I will fired from my job. Also, this a problem at home too, I am constantly doing the opposite even after I agreed that I wouldn't. I feel sad and lonely most of the time and I don't like to leave the house.
