My mother has soldier pain when she twists her hand backward. It was started when she was washing clothes 3 months ago. Does she need to do xray?

A pain associated with shoulder rotation usually indicates a rotator cuff pathology. A through examination should help ascertain the same. X-rays are seldom of use in cuff related issues.
Next Steps
have her shoulder examined by a orthopedic surgeon
