Hello Doctor, I would like to discuss my concern regarding my pregnancy. During the TVF scan, internal bleeding was noted. The pregnancy is measuring 6 weeks and 2 days, and the sac size is 4.9 mm. I would like to know if there is still a chance for this pregnancy to continue, or if this indicates a possible miscarriage. In case of miscarriage, will a D&C be required, or can it resolve naturally on its own? Kindly advise.

This scan reveals a pregnancy that is quite early. The pregnant sac is bleeding slightly, which is typical in the early stages of pregnancy. Even when the sac is smaller than anticipated, a single scan cannot yet establish a miscarriage.
Next Steps
After seven to ten days, a follow-up scan is required to determine whether the pregnancy is progressing and whether a heartbeat is seen.

It must be sub chorionic bleeding inside.IF It's a small size nothing to worry much as it will resolve eventually.Provided the size of bleeding is small it can be followed up in subsequent scans
