Red patches on the tongue

Should I see an ENT doctor or oral maxillofacial surgeon for this? Have this red patches on top of the tongue back side for an year now. Since March 2022. It's painful and sore constantly. Do I need to get a biopsy? Does it look cancerous since I have it for more than an year. Is this something serious? I am so worried and anxious about it. Whom to see further for this.

Yes consult an ENT and head &neck surgeon. Needs proper examination and biopsy for accurate diagnosis. Since it's there for an year , it's unlikely to be a malignancy. But can't rule out without biopsy.
