Recurring anal fissure
Hi all
I have been having issues with anal fissures since 2014. In 2014 a sigmoidoscopy was done and it indicated healing anterior fissure and a posterior fissure in ano. After that, I've been having 1-3 episodes of fissures since then. However, in Jan 2019, I had a very bad case of constipation and that brought back the fissure and it took almost 18 days to heal and stop bleeding. The fissure has been more reccuring now.
Jan - 18 days
Feb - 2 days
March 5 days
May - 3 days
June - 4 days
August 4 days.
All the times at the same spot. It is on the anterior end but not exactly on the midline (picture attached). What can I do and how do I make sure healing happens? Thank you

This is not a Fissure.
Just because there is abrasion in anal mucosa doesn't make it a fissure.
Sigmoidoscopy is not needed to Diagnosis a fissure. A fissure has some characteristics which this appearance doesn't have.
If there is any linear abrasion shown in sigmoidoscopy doesn't mean it is a fissure.
These red lesion need DETAILED evaluation. The abrasion visible in Between two cushions is probably because of some other pathology.
Biopsy may be needed to rule out Inflammatory bowel disease also as the lesion is not typical.
