I am a 29-year-old woman with recurrent culture-proven UTIs since 2021, initially mostly E. coli ( 6months gap). Post IBS in 2024 proteus and latest Enterobacter cloacae were detected and gaps reduced to 3 months. After antibiotics dosage completion  burning urination returned after 2-3days . These episodes were culture negative with epithelial cells >15/hpf. Most recently, culture showed E. coli >100,000 CFU/mL. Tamsulosin 0.4 mg and microlift b6 significantly reduces discomfort . USG, PVR, uroflowmetry, MCU and DMSA are all normal. One urologist suggested UDS, while another felt my dysuria was because of somatization and advised against UDS. Why do I keep getting different bacterial UTIs despite reassuring tests, and why does dysuria increased post ibs? Could altered vaginal flora, urethral sensitisation or another factor explain this? Also is there any possibility to get -ve culture even if any bacteria present are dorment/biofilm? I did take cranberry supplements as well.
