Reccuring Varicocele pain

Hi. I was diagnosed with grade three left varicocele three years ago and had a open surgery for the same. After six months the varicocele returned and I was diagnosed with bilateral varicocele with left grade three and right grade two. I have recurring pain on my left since a month. what do I do.
