Rashes marks on whole body and itching

Dear Doctor, i can't understand the reason and issue what's the reason. My wife is having Itching in night mostly may be that we get free from work and concentration goes there. When her body get wet by water or sweat  facing the itching problem. Rashes are spread over full body like hand, leg, ankle, back, stomach etc. When body is dehydrate then also i face itching issue. Dryness form itching & after scratches over the effected area bleeding has been started. Sharing the pictures of rashes please suggest and help
