You have asked several fairly technical questions best answered after detailed examination of your eyes.
1. We can differentiate regular astigmatism from keratoconus by careful slit lamp exam along with measurement of corneal thickness ( pachymetry) and id in doubt by an investigation called topography.
2. The signs you mention are seen in moderate keratoconus
3. All eye specialists are trained to do this. Many later train for different sub specialties like retina or oculoplasty and hence may not appreciate these changes as much as cornea specialists may.
