ICT can be positive even after Anti D. There may be 2 reasons for this, one is the Anti D injection itself, do a qualitative test of Anti D, if it 1:4 dilution or less, that means the test is positive because of the injection and you have nothing to worry about. Second reason could be that Anti D injection was unable to prevent immunisation from happening, which happens if the feto maternal bleed is large. In this case, the quantitative levels of anti D test will be high, and your doctor will amnage them accordingly.
