Please guide

I don't know what's happening with me these days 5 months ago before I started gym I was in depression along with anxiety now I am studying for medical entrance exam.. Something which can't even be considered as problem bothers me a lot my heart starts beating very fast and I over think a lot over small thing. I still feel the anxiety. Please guide me what should I do I feel low and hopeless.
