Penoscrotal webbing problem

I am circumcised since childhood.I was circumcised until  half of the penile shalf.I have been feeling irritation near penoscrotal skin. It has extremely loose skin. The skin of testicles goes all the way up till penis shift which makes difficult to wear condoms. And also it is causing downward bend. Please refer me treatment and diagnose real cause of problem.
