I have some spots over my penis since the age of 15 and its still same as before after 20+years. I once see doctor for this he told me it's vitiligo but when i compare mine symptoms with penile melonosis and its look it match better than vitiligo. Now i odd about it and needs to get it remove with treatment or surgery. Any recommendations or suggestions?

Yes it is penile melanosis. First thing you have to understand is With any procedure you can't get normal looking skin. With non surgical procedures like lasers and creams total clearance may not be achieved. With surgical management, skin grafting should be done. In both cases you won't have normal looking skin.
Next Steps
consult a dermatologist for non surgical management or a plastic surgeon for surgical management directly.
