It can be due to anal fissure, hemorrhoids   or it can be  due to constipation so it would be better if u visit doctor (surgery)

Actually piles don't cause pain. They only cause bleeding. When piles develop  complications then they become painful. Apparently you have not been seen by any doctor.
Next Steps
Please consult a General Surgeon in your area. There should be atleast one physical consultation before you start doing tele-consultation. After all at least the diagnosis should be confirmed by a General Surgeon.
