Meet a pediatric dentist and please get a radiograph to see the extent of the decay. If your son is complaining of pain then it surely indicates the decay has progressed deep and needs a root canal therapy which is very safe to undergo if you are apprehensive about it

Hello sir, if your son is complaining of pain, better to do pulpectomy procedure for that teeth... Since he is just 4yrs old, his permanent teeth will start erupting after 6yrs only, so he can't bare the pain for so long... But do pulpectomy and save the tooth... Pulpectomy is nothing but a filling which is done till the root of the teeth...

Please get him checked. If the filling is broken it may require a new filling. If filling is intact but tooth is painful then pulp capping or pulp treatment may be required.
